Every form the guides on this site talk about, in one place. Tap a form to open it; how to fill it out is explained in its guide.
NYC (NYPD) Application
How to fill out and upload each form is explained in Step 3: The Forms of the NYC (NYPD) Concealed Carry Guide. Applying from outside NYC? See Step 3 of the Non-Resident NYC (NYPD) Guide.
Save each PDF, fill it out, then print and sign it (and notarize it if needed). Scan the signed copy and upload it to the NYPD portal.
Forms to Fill Out and Upload
Affidavit of Co-Habitant (PDF) — One for every adult who lives with you, and each one must be notarized. If you live alone, fill out the bottom part and sign it; no notary needed.
Acknowledgment Safeguard Form (PDF) — Filled out by the person you choose to safeguard your firearms in the event of your death or incapacity. It needs a witness, not a notary.
Affirmations - Penal Law Article 35, Article 265 & Article 400 Form (PDF) — Your signed statement that you have read and understand Penal Law Article 35 (use of deadly force), Article 265 (criminal possession of a weapon) and Article 400 (responsibilities of a handgun licensee).
Affidavit of Familiarity with Rules and Law (official NYPD copy) — Must be notarized. Might not be required, depending on the investigator, but I recommend uploading it.
NYPD Affidavit of Character Reference (PDF) — Can be used instead of a character reference letter; you need one or the other, not both. Each reference must be signed and notarized. Prefer a letter? Use the sample character reference letter in the NYC guide.
New application: 4 character references. Renewal: 2.
Interview Questionnaire (updated 3/17/2026) — This form will be handed to you at your fingerprinting. Answer every question truthfully. Any YES answer must be explained on the form's Additional Information page.
Question 21: Yes.
Question 21.A: No (unless in your case it is yes).
Question 26: "When unable to retreat to a safe location, when being faced with deadly force and fear of imminent death or serious bodily harm to myself or to my loved ones."
This is the site owner's guidance, not official NYPD wording and not legal advice.

How to Get a Certificate of Disposition (PDF) — From the Legal Aid Society: where to get a Certificate of Disposition in each NYC borough and what to bring. If you were ever arrested, the NYPD needs one for each arrest.
